Psychiatric Care Alongside Home Health Services
I-HPS works alongside home-health teams to provide psychiatric evaluations and ongoing medication management through secure video visits throughout California. We assess mood, anxiety, thought, and cognitive concerns in the context of the patient’s medical conditions, recent transitions, and daily support needs.
Our role is outpatient psychiatric care. We collaborate with home-health nurses, primary care clinicians, specialists, and caregivers when appropriately authorized, while the existing medical team continues to manage the patient’s medical and home-health needs.
Precision Medication Management combines a whole-person psychiatric assessment with symptom tracking, medication and supplement review, and individualized laboratory monitoring. We consider prior treatment, sleep, diet, movement, and patient goals. PGx testing may be considered when clinically appropriate; results can inform certain medication decisions but do not guarantee treatment response.
Relevant medication lists, laboratory results, discharge summaries, and caregiver or nursing observations help inform assessment and follow-up. We review treatment response, functioning, and side effects over time and coordinate care with the patient’s permission or appropriate authorization.
New patient intakes are available within 5 working days. Self-pay fees are $300 for the comprehensive initial assessment and $150 per follow-up appointment. PGx and laboratory tests are billed separately, with costs discussed before ordering. Our office verifies insurance coverage and any referral or authorization requirements individually.
